In a patient with ulcerative colitis and primary sclerosing cholangitis undergoing yearly colonoscopy surveillance, current UK guidelines recommend continuing the use of colonoscopy with chromoscopy (dye spray) for mucosal assessment rather than substituting it with Texture and Colour Enhancement (TXI) mode on the endoscope. This is because chromoendoscopy with dye spray remains the established method for enhanced detection of dysplasia and mucosal abnormalities in inflammatory bowel disease surveillance, particularly in high-risk groups such as those with primary sclerosing cholangitis. There is no recommendation or evidence in the current NICE or British Society of Gastroenterology guidelines supporting the replacement of dye spray chromoendoscopy with TXI mode for this purpose.
