Contraindications for the use of medroxyprogesterone acetate include:

  • General contraindications: acute porphyrias and current breast cancer (unless progestogens are being used in the management of this condition).
  • Specific contraindications with oral use: history of thromboembolism and undiagnosed vaginal bleeding.
  • When used for contraception: current or history of meningioma.

High-dose medroxyprogesterone acetate (all parenteral preparations and oral preparations containing 100 mg or more) is contraindicated in patients with meningioma or a history of meningioma; if meningioma is diagnosed during treatment, the drug must be stopped.

These contraindications are based on safety concerns including thromboembolic risk, cancer risk, and undiagnosed bleeding that may indicate underlying pathology.
